COVID19 - CHECK IN QLD APP - A legal requirement before entering Unique Blendz Studio
Thank you for registering to use the Check-In Qld app in our venue and for your ongoing support of the COVID-19 contact tracing efforts.
The Check-In Qld app will enable you to easily comply with the Public Health Directions by enabling you to self-check-in at our studio and have your information directly stored with Queensland Health for contact tracing (if required).
Before entering the studio, please click the below link to download the App on your phone.
Once you enter the studio, you will simply scan a QR Code to register your details at our venue. The Queensland Government App will request your full name, phone number, email address & street address (optional).
If you do not have a smart phone/not familiar with technology or mobile data to do this, we will have a device at the studio for you to use or our staff can do it manually for you.
Please ensure you arrive 10 minutes prior to your appointment to commence all paperwork & numbing. If you are running late, your appointment may need to be rescheduled as adequate time is needed to perform treatment. *Please note your deposit may be lost.
Due to health and safety reasons , please do not bring children or groups of friends to your appointment. The treatment room is for the technician and client only. As you can understand, 100% focus is required on you and we do not want to compromise your treatment.
To book any appointments, you must pass all Cosmetic Tattoo Eligibility Questions, be able to adhere to all Pre-Procedural & After Care information. If you do not adhere to all information, this will heavily effect your results. If you arrive to an appointment not adhering to pre-procedural information, Katelan can cancel your appointment and your deposit will be lost.
If there are any active cold sores, acne, moles or blemishes where the procedure is to be performed, Katelan must be notified immediately as your appointment may be postponed for a future date once blemishes have cleared or moles have been removed and healed.
If you are pregnant or breastfeeding please advise Katelan and treatment will have to be postponed.
Please be aware that all cosmetic tattoo treatments are an art rather than a science. All skin types heal differently and results can vary from client to client. Unique Blendz cannot guarantee successful results on 100% of our clients as everyone’s skin heals and accepts the pigment differently. Prospective clients should review all information contained on this website and during the pre procedure consultation as we do not offer refunds on deposits or treatments.
A $50 booking deposit is required to book any New Client treatment at Unique Blendz. Without a deposit, no appointment time or date can be confirmed or secured. Touch up appointments require a $30 booking deposit.
Deposits are strictly NON-REFUNDABLE or TRANSFERABLE to another person. Please ensure you read all information thoroughly before securing with a deposit.
Deposits are to be made via the online booking system. No bank transfers or cash is accepted.
Deposits will be deducted from the total price that is to be paid on the day of the procedure or put onto an interest-free payment plan.
* If you are booking 2 x appointments, 2 x deposits will need to be paid to secure any dates and times.
If you have a treatment secured, you will receive an SMS Confirmation 4 days prior to your appointment. You must respond to this on that day or if you can’t please contact Katelan on 0435 325 248 to confirm. If no response is heard, your appointment will be cancelled & filled by another client.
PAYMENT PLAN POLICY
Unique Blendz offers Afterpay on all cosmetic tattoo procedures. After the booking deposit are secured, the remaining balance can be finalised using Afterpay. If you wish to use this service, please ensure you have signed up to Afterpay & downloaded the app & checked your credit limit PRIOR to arriving at Unique Blendz.
If you need to reschedule your appointment, this must be done at minimum 7 days prior to the appointment and your deposit will not be lost. If less than 7 days notice is given, the deposit will be lost & another booking deposit will need to be paid to reschedule your appointment.
*Please be aware, if you do have to reschedule, as Katelan is heavily booked, it may be a 2-3 month wait to get another appointment. Please be certain of dates and times when booking appointments.

NO SHOW POLICY
If you do not show for your appointment and no notice is given, your deposit will be lost.
Before your next appointment is booked, you must pay half of the total price upfront prior to the treatment.
Unique Blendz reserves the rights to refuse its services and prices are subject to change at any time.
If you have had your brows cosmetically tattooed before, treatments are quoted individually based on existing pigment, shaping and quality of the skin. Prices will differ from client to client.
EXISTING BROW TATTOO
If you have an existing cosmetic tattoo in the area you wish to be treated, photos must be sent to Katelan on 0435 325 248 PRIOR to booking. The work must be assessed first for approval.
Unique Blendz can refuse service if your existing brow tattoo can not be covered. In some cases, if initially tattooed incorrectly, clients may need to commence removal treatments prior or let the pigment fade over time before getting their brows re-done. Ideally, pigment needs to be faded by 80% to refresh the colour.
Unique Blendz does not do 5-8 week touch ups on non-existing clientele as each technician uses different brands of pigments and artistic style. If you require this, please contact your initial technician.
